Preventable medical error

The Institute of Medicine’s landmark report, To Err is Human: Building a Safer Health System, brought into focus the preventable medical errors that are negatively affecting patient outcomes 1).

Preventable medical errors in the neurosurgical field are of particular interest, given the 2) 3) 4) potentially fatal consequences. Among the factors contributing to this problem is that variance 5) 6).

Among the factors contributing to this problem is that variance between providers exposes the healthcare system to inefficiencies and worse health outcomes 7) 8) 9) 10) 11).

There is a need for the systematic accumulation of evidence to support clinical decision-making 12)
