Type I fractures are due to avulsion of the attachment of the alar ligament. They are very rare. Although long considered to be a stable injury, they may not occur as an isolated fracture and may be a manifestation of atlanto-occipital dislocation.

If there is associated atlanto-axial instability, surgical fusion may at times be necessary.
