When extracranial carotid stenosis produces a transient ischemic attack or stroke, artery-to-artery embolism is the predominant mechanism. Asymptomatic significant (>50%) carotid stenosis poses special clinical questions in patients scheduled to undergo general surgical or major cardiovascular operations.
