Crowned dens syndrome is a rare presentation of calcium pyrophosphate deposition disease. It is characterised by severe neck pain and neck stiffness. Acute presentations are typically accompanied by fever and an inflammatory response and hence can be misdiagnosed as polymyalgia rheumatica or meningitis.
