For a combined supra- infratentorial posterior transpetrosal approach the incision should expose a large part of the lateral aspect of the temporal bone and, partially, the suboccipital bones. This is why we use a question mark incision starting three fingers above the zygomatic process, moving posterosuperiorly and posteroinferiorly surrounding in a semicircular fashion the pinna, descending until two fingers below and behind the mastoid tip.
