SHE Score (Swelling, Hemorrhage, Expansion Score) The SHE Score is a prognostic tool used to assess the risk of neurological deterioration and poor outcomes in patients with [[intracerebral hemorrhage]] (ICH). It evaluates three key factors that contribute to secondary brain injury: Components of the SHE Score Swelling (S) – Perihematomal Edema (PHE) 0 points: Small or no edema 1 point: Moderate edema 2 points: Severe edema Hemorrhage (H) – Volume of Intracerebral Hemorrhage (ICH) 0 points: < 30 mL 1 point: 30-60 mL 2 points: > 60 mL Expansion (E) – Hematoma Expansion 0 points: No or minimal expansion 1 point: Moderate expansion 2 points: Significant expansion (> 6 mL or > 33% increase) Total SHE Score: 0 to 6 Higher scores correlate with worse outcomes, increased mortality, and greater risk of neurological deterioration.
